Alternate Destinations

Minnesota Law states the school district must have a written agreement with parents of a student to allow the district to pick up or deliver a child to a destination other than the parents' legal residence. Please return the completed form to American Student Transportation at

The  Alternate Destination form is to use if your child will be picked up or dropped off by the school bus anywhere other than home for reasons such as childcare.  (You do not need this form if your child is picked up and dropped off only at home.)

Temporary Transportation Change From (PDF - 34.33 KB)

 

Use this form only for a temporary, short-term change to your child's bus stop, due to daycare needs.  The bus company needs 48 hours notice for any stop changes, so if you have an immediate transportation emergency, please contact the school office.  (This form is not be used for play dates, sleep-overs, etc.  Parents must transport to those types of after school activities.)
